ASSESSMENT BOOKLET
This assessment booklet should be used one per student for
the entire year. Print and store one copy in each student’s
assessment file. Teachers can make extra copies of
particular pages when necessary. Consider these
instructions a guideline that can be adapted to your
teaching approach.
Page 1: Intro — Activity can be completed in the first week of school.
Page 2: News writing — Multiple copies to print throughout the year to
track drawing, handwriting, storytelling and grammar progress.
Page 3: Letter writing — Multiple copies can be printed for termly or
monthly checks; teachers call out letters for students to write.
Page 4: Name writing — One-page needed to record termly name
writing with space for teacher comments.
Page 5: Number writing — Termly samples on one page to show
progress.
Page 6: Reading log — Use per teacher preference to record termly
reading levels and comments.
Page 7: Yearly teacher observations — Notes to support parent–teacher
conferences.
Page 8: Early finisher activity — Quiet task for students who finish early
and during assessment time.
